  Job Description

This role is working with adults with a mental health diagnosis. Based in a busy community mental health team in Deptford. Based both on site and in the community. Some opportunity to agile work but expectation is to be available to be seeing service users 5 days, Mon-Fri 9am-5pm. Role is primarily (but not exclusively) to support discharge and FLOW. The work includes supporting those on your caseload to attend community appointments that will offer future support, supporting medication arrangements on discharge and working with the care coordinator in keeping care plans and other documentation up to date. Support around care act assessments and lots of carer contacts very much expected. Busy, lively and lots of support provided by the team. Regular supervision is mandatory.
